All recipients of our furniture have been referred to us by other Agencies or Organizations such as the Department of Social Services, Minnehaha County Human Services, Center of Hope, The Community Outreach, Lutheran Social Services, Children's Inn, Churches, Schools, and Hospitals.
If you are a member of one of these agencies and you are aware of someone in the community who has a need for clean, "gently used" furniture, please fill out our online referral form below. If you do not know the password, please give us a call at (605) 977-6800. *Faxed referrals will no longer be accepted after August 1st.
Referral Process Steps for Referring Agencies:
1. Complete Online Referral Form - if you do not know the password for the referral form, please call (605) 977-6800. Passwords should not be shared.
2. Clients must sign the Sioux Empire Network of Care Consent Form. All adults need to sign a separate form. If an adult in the househould is not present, please indicate on your referral form. Furniture Mission will follow up with the adult for consent. Fax the signed consent forms to the Furniture Mission.
3. Please remind clients they need to follow up with the Furniture Mission in 2 to 3 days to check the status of the referrals. Once approved, the referral stays open for 6 weeks.
Furniture items are delivered to our clients free of charge, and all referrals are treated with the utmost in discretion.
